  Probation Plan  
 
  1. The Probation Plan consists of additional rules and conditions of probation the offender must adhere to during supervision.
  2. The Probation Plan should identify the needs of the offender for substance abuse treatment, mental health treatment, employment skills training, educational needs or other activities that will assist the offender with being successful on probation.
