Convalescent Care is provided for homeless persons who are being released from the hospital, but are not yet physically ready to care for themselves.  From the hospital, they go to a nursing home to receive basic maintenance / convalescent care for a short time.  During their stay, they are provided with information about services that might help them to make changes to their lifestyles. They are also assisted in accessing those services (if that is their choice)

Things you should know:

1. Convalescent Care is provided ONLY upon referral from the hospital social worker, prior to discharge.
2. Convalescent care is not long-term care.
